We are hiring for an: Outreach Case Manager
Plateau Outreach Ministries (POM) provides social services for persons in the area of southeast King/Northeast Pierce Counties commonly known as the Enumclaw Plateau. At the end of 2019 POM was awarded a contract by King County through their Veterans, Seniors and Human Services Levy (VSHSL). POM is actively seeking hiring a Case Manager to help us fulfill our contract with VSHSL and meet the need of the clients we serve in the Plateau. This position is partly funded by the VSHSL funding.
Overview: This is a 30 hour a week case management position which focuses on outreach to Seniors, Veterans, and Resilient populations within POM’s service area. The Case Manager will provide information, resources and case management to help clients leverage community and government benefits to achieve financial stability. Given the diversity of our operations, it will be necessary to assist where needed to best serve the organization. This will occasionally require duties outside the listed primary duties. Role:
· Reports to the Executive Director
· Reaches out to Seniors, Veteran and Resilient population by providing community presentations, resource booth, etc.
· Establishes relationships and network with agencies that provide resources to Senior, Veteran, and Resilient populations
· Maintains client confidentiality
· Ensures POM’s Intake paperwork is completed
· Provides assistance to clients throughout the entire process, from applying for resources/benefits to receiving the final decision, and ensuring the client has access the support that best meets their needs
· Uses active listening and interview skills during case management sessions with clients
· Advocates for clients as they navigate resources systems walking them through all the steps until the outcome is provided
· Attends VSHSL meetings and trainings, as advised by supervisor
· Collects, enters, and maintains data that meets POM and VSHSL requirements
· Follows POM’s policies and procedures
· Works and trains staff/volunteers to assist when needed
· Becomes a certified case manager with Washington Connection and assists clients in applying for state benefits (training is provided)
· Assists with food bank weekly to help build relationships with clients
· Works as a team with the director, other staff and volunteers
Additional Information:
Additional duties may be added over time to best serve the organization. Given the variety of activities throughout the week and over the course of the year, it is important to be a team player and be willing to assist where needed. The employee may be expected to lift up to 30 pounds. The employee is required to maintain confidential information. The employee may be required to interact with individuals who are under extreme stress and individuals with special needs. The employee is required to maintain professional composure and tact, patience and courtesy at all times. The employee is expected to have a verbal and visible commitment to the Vision, Mission and Values of POM, as well as a commitment to represent churches of the Plateau Ministerial Association.
EXPECTATIONS AND QUALIFICATIONS:
· This is a part-time position with a flexible schedule in order to meet outreach and client requirements and needs of POM
· Must have experience in working or volunteering in a Social Service organization that provides services similar to POM
· Have experience with providing outreach, public speaking and/or staffing resource booths
· Must have a working knowledge of local, county, state, and government resources in order to assist clients in crisis
· Must agree to a credit check and pass the WA State Patrol and/or a federal background check
· Must have reliable transportation and is able to accurately track mileage for reimbursement
· Must be a self-starter with high integrity
· Must be able to work as a member of a team
· Bilingual in English and Spanish is a bonus
COMPENSATION:
The rate of pay will be dependent upon the employee’s education and experience between $20-$22 hr. All jobs at POM are considered temporary for the first 90 days. After 90 days have been completed, you will be evaluated and may be offered the permanent position. Permanent Staff who work over 24/hrs. a week receive paid holidays, vacation days, up to 3% match in a SIMPLE IRA, and a monthly medical stipend. All staff receive sick leave.
